Our online patient portal provides you with secure and convenient access to your personal health information (PHI).

The patient portal allows you to:

  • View lab or radiology results – Your lab results will be available on your portal within 5 business days of your visit. We will still call you to discuss and review your results!
  • Check parts of your medical record
  • Upload documents for your provider team to review
  • Communicate with your provider teams
  • Check your referrals

Register for the Patient Portal

If you would like to register for your portal today OR are having issues registering, logging in, or resetting your password, please email [email protected] with the following information:

  • First and last name
  • Year of birth
  • The email address you would like connected with your portal to receive notifications
  • Reason for email: Portal registration or portal issue

Our Patient Portal is a courtesy for patient convenience. Gwinnett Clinic’s Patient Portal is NOT meant to be used for urgent, emergent or time-sensitive situations. Communications should be limited to informational, refill or appointment requests and clinical requests that can be responded to within 5 business days.

In case of an emergency, call 911 or go to the nearest emergency room. The best way to reach your physician or NP regarding a time-sensitive but non-emergent clinical issue is by calling the office during business hours and scheduling or walking in for an appointment.

Our Patient Portal should be used for:

  1. Viewing Lab Results
  2. Viewing Radiology Reports
  3. Viewing Specialist Referrals
  4. Prescription Clarifications (if seen within the last 3 mos)
  5. Uploading Documents
  6. Other Non-Urgent Communicaiton with your Care Team
It should NOT be used for:
  1. Scheduling/Requesting Appointments (call or request online)
  2. New Medical Issues (call or request online)
  3. Medical Records Requests (request online)
  4. Customer Service Issues (email or call Care Coordination)
