How can I schedule an appointment?

- In-person at any Gwinnett Clinic office.
Even if you do not have a scheduled appointment, our primary care physicians are always happy to see you as a walk-in patient. Our specialists require that you schedule an appointment, but same-day or next-day appointments are usually available.
We highly recommend that you call the location that you plan to visit and schedule an appointment with your preferred physician because they will have the most up-to-date information about physician schedules and office hours.
Helpful tips for your visit to Gwinnett Clinic:
To schedule an appointment, please call one of our 17 locations.
- Bring a valid photo ID and your insurance card / information.
- Expect to pay co-pays / visit fees before your appointment.
- If your insurance coverage has changed, please bring your updated insurance information.
- If you do not have insurance, a physician or nurse practitioner may still be able to see you. Call ahead to find out about affordable rates for you.
- All new and established patients fill out medical forms prior to their appointment.

Health Insurance
We accept most common commercial insurance plans (HMO/POS/ PPO), traditional Medicare, workers’ compensation, auto insurance, and indemnity insurance. Our list of accepted insurance providers is subject to change at any time. Please contact your insurance company to confirm Gwinnett Clinic is in-network before scheduling your appointment.
If you do not have insurance, our physicians are happy to see you as a private pay patient.
Medicare:
For 2025, Gwinnett Clinic will continue to welcome traditional Medicare and supplemental plans but is NOT accepting Medicare Advantage plan patients. Please call your Gwinnett Clinic office to confirm we are in-network if you are uncertain.
Medicaid:
We are not accepting new Medicaid patients at this time. For current Medicaid patients, our specialty physicians do not accept Medicaid.
Medical Records
For all medical record requests, please ensure the patient’s name and date of birth are legible on the Medical Records Request Form. The patient’s signature is required. Requests without the patient’s signature cannot be processed.
If you are clinical staff from a hospital or doctor’s office and have a Gwinnett Clinic patient in the office or scheduled within the next 24 hours, please call 770-765-1101. This number is for clinical staff for urgent care coordination only.
If this is a billing-related inquiry for records, please fax your written request with the appropriate signature to Patient Financial Services / Billing at 678-373-3942.
We will do our best to get records to you in a timely manner in accordance with state law.
We welcome patients from most commercial health insurance plans and traditional Medicare.
