Scheduling Your Appointment Is Easy!

We have three convenient options for you to schedule your appointment:

1. Call our appointment line at 770-995-3300
2. Call any of our 17 Gwinnett Clinic locations. Same or next-day appointments are usually available with our primary care physicians by phone only. Appointments to see our specialists may be available within 1-2 days by phone as well, based on availability!
3. Complete the form to request an appointment with your physician at Gwinnett Clinic. Online appointment requests should be made at least 72 hours in advance.

Please note, this form is a request only. You will receive an email or phone call to confirm the appointment. If you do not hear from us within 48 hours of submitting your request, please call the Gwinnett Clinic location where you would like to be seen.
